The forms should be printed out, filled out LEGIBLY, and include the student's full name, full dates of the clerkship completion (day, month, year), the signatures of the tutor and the head of the department, their personal stamps or names written legibly in capital letters, a stamp bearing the name of the department/facility or, if the stamp is not available, the full name address of the facility written legibly in capital letters.
Each summer clerkship form should be submitted to the Dean's Office each year immediately after its completion.
